Referral Process

Helping clients connect with the right behavioral health support

Our referral process is designed to help individuals access services in a clear and supportive way. Once a referral is received, our team reviews the information and helps connect the client with appropriate care.

Who can place a referral?

Referrals may be submitted by clients themselves, family members, healthcare providers, community organizations, schools, attorneys, case managers, social workers, hospitals, agencies, or other professionals assisting someone in need of behavioral health services.

A client may also contact us directly if they are seeking mental health counseling, group therapy, family support, case management, mental health evaluation, immigration mental health evaluation, substance abuse services, or substance abuse evaluation.

When someone other than the client submits a referral, we will still work to ensure the client is appropriately contacted, informed, and involved in the next steps of care.

What happens after a referral is submitted?

After the referral form is completed, the information is reviewed by our team. We use the referral details to better understand the client’s needs, requested services, availability, and any important concerns that may help with care coordination.

Our team may contact the client or referral source for additional information, confirm the requested service, review next steps, and help determine whether BWellness Behavioral Health is an appropriate fit for the client’s needs.

Our team reviews the referral and contacts the client or referral source to gather any needed information.

Connect to Care

The client is guided through next steps, provided access to the client portal, and assigned a clinician when appropriate.

Client portal access

Once the client is accepted for services and the next steps are confirmed, the client may receive access to a secure client portal. The client portal helps support a smoother intake and care experience.

Through the portal, clients may be able to complete intake forms, review practice documents, provide required information, receive appointment-related communication, and stay connected with the clinic.

The portal helps our team gather important information before services begin so the assigned clinician can better understand the client’s needs and prepare to provide thoughtful, individualized care.

Clinician assignment

After the referral is reviewed and the client’s needs are identified, the client may be assigned to a clinician who can best support the requested service and presenting concerns.

Clinician assignment may consider the type of service requested, clinical needs, availability, scheduling preferences, and the client’s goals for care.

Our goal is to match each client with appropriate support so they feel heard, respected, and connected to services that can better serve their needs.
